Registering my iPAD AED

Before you'll be able to complete this form, you will need the following pieces of information to hand:

1) Your Defibrillator's Serial Number.
This can be found on the back of the defibrillator, or on the box that the defibrillator came in.

2) Cabinet Details.
If you have a cabinet, you'll need to know it's unlock code (if it's locked) as well as it's model.

3) Location Details.
In order to let the ambulance service know where your defibrillator is, we'll need to know the exact location of your defibrillator. It's advised you have your defibrillator in place before you complete this form.

4) Guardian/Owner Details
We will require your details, as well as the Guardian's (the person who is responsible for looking after the defibrillator) details.
